NOTE: If a provider requests permission to use a tool not on this list, the CFC should submit the request

to their developmental pediatrician consultation contractor. The developmental pediatrician consultation contractor

should review the tool and submit their findings and recommendations to DHS for a decision about

whether or not to add the tool.  DHS will then notify all CFCs regarding the final decision.
