Documentation is critical to ensuring thoughtful, planned service delivery and can be used in a variety of ways, ranging from capturing a family’s story to meeting criteria for demonstrating outcome achievement and fulfilling credentialing requirements. However, documenting service delivery and family progress can be challenging for program staff. This training introduces a systematic process for developing and maintaining service records. The training focuses on the skills needed to record observations and interventions during home visits and suggests techniques for writing annual plans for group services, as well as individual session plans and follow-up notes. Included in this process are strategies for working with teen and young parents to determine their strengths, needs, capabilities, and aspirations so that each individual family support plan includes relevant short- and long-term goals and can be used to track family progress.
