Illinois Coordinating Care Between Early Intervention and the Primary Care Medical Home

Description Developed by a collaborative project of the Illinois Chapter of the American Academy of Pediatrics, Advocate Health Care's Healthy Step Program, the Illinois Bureau of Early Intervention, and the Early Intervention Training Program at the University of IL, this project was supported by a grant from the Commonwealth Fund.

This resource is intended for an audience of Early Interventionists, especially those that work with the Individuals with Disabilities Act, Part C, and provide services for infants and toddlers in Illinois.
Presented in two 20 minute modules, Its goals are to improve the delivery and financing of preventive health and developmental services for children birth to three and to build on existing programs to develop a range of strategies for primary care settings.

Participants must complete the training module, successfully pass the quiz, and complete the required training evaluation inn order to receive a certificate of completion.
