Description

DSCC’s mission is to partner with families and communities to help children with special healthcare needs connect to services and resources. Through this partnership, DSCC strives to put children and youth with special healthcare needs and their families at the center of a seamless support system. DSCC care coordination teams carefully assess each family’s unique needs and develop care plans to help them achieve their goals. This support empowers families and helps them reach their full potential. This presentation will guide participants through DSCC care coordination services and provide an overview of program eligibility.
