Service Coordinator's Role As Families Exit Early Intervention

Service Coordinators play a pivotal role in facilitating a supportive transition as families leave the Early Intervention system. The responsibilities of the Service Coordinator include interacting with parents to identify the needs and goals of the family going forward, completing necessary paperwork and documentation, and coordinating with other professionals and organizations (e.g., school programs, other state early intervention programs and DCFS). The reason families exit the program vary: the family moves, the child is found to be ineligible for our program , the child has met program objectives, the child is aging out, and the parents decide to leave the program. This workshop will provide a refresher course to review policy and procedure, the meanings of closure codes, and important communications with parents and community partners(e.g., EI providers, school programs).
