The transition from the Infant Toddler program to School-Based services includes many changes for the various stakeholders and lends itself to both excitement and stress. A change in philosophy, moving from a family-centered to a student-centered approach, involves transitioning from a primary service provider model to multiple providers and from year-round services to a school calendar. The laws pertaining to transition do not emphasize the importance of collaboration between the sending and receiving agencies; however, studies have identified how collaboration and team support positively contribute to transition outcomes. This session will describe the legal requirements according to IDEIA, review current research, and describe strategies and supports used to ensure a smooth and seamless transition for children turning 3. Barriers and facilitators related to the successful transition between the home and school contexts will be explored using case examples from a large metropolitan school district. The session will end with team-based learning exercises, including developing an IEP with participation-focused SMART goals and a discussion of dosing strategies for children transitioning from EI to preschool.
