Recording the Journey: Documentation and Case Notes in EI

Description

The Child & Family Connections staff are mandated to create and maintain a record that tracks events for every child referred to early intervention. This legal record captures the story of the family/child that permits a primary mode of communication among team members. Maintaining high quality case notes ensures that children and families are best served even when changes in the service coordinator occur (e.g., assigned Service Coordinator is unavailable; families move to a different service region) and provide the support and justification for services and actions. Compliance with agency procedures dictates timely documentation that is thorough (i.e., all key events are noted), professional, accurate, clear and concise. This workshop will review the policies and procedures for documentation in the EI System and support participants in an examination of their own documentation habits/practice.
