Facilitating the Development of Successful IFSPs from Intake to Direct Services

Description

This training was developed to enhance Service Coordinators’ knowledge at CFC 25 about service plan development to ensure that the IFSPs are reflective of families’ concerns and priorities. In small groups of three to four service coordinators, we will review completed service plans starting with the information gathered during the intake process and evaluations, and reflecting upon how it is being integrated into the service plan. Strong focus will be placed on family assessment, how it is completed and then communicated to evaluators and the IFSP team.
