Early Intervention Transition Policy/Procedures

The Illinois Department of Human Services identifies policy and procedures for transition activities.

Transition Training Modules

Online training modules that provide information on the goals of transition, options available to children and families when they leave early intervention, and the responsibilities of all who are involved in the transition process.

  • Transition is an online training module from the Illinois Early Intervention System Overview that has been made available as a resource for individuals to review. 
  • Transition: Partnering with Families and Supporting Children as they Leave Early Intervention is an online training module that was developed with the intent to provide some universal information on transition for families, early interventionists, and local education agencies….regardless of where you live and/or serve children and families. This module will reflect information from Part C (early intervention) and Part B (special education) in the federal law IDEA (Individuals with Disabilities Education Act). Please remember to check your local policies and procedures for additional specifics that may apply to transition in the state in which you reside.

State of Illinois Infant/Toddler & Family & Family Rights under IDEA for the Early Intervention System

This resource describes a child's and family's rights, as defined by Part C of IDEA, and includes provisions for early intervention services for eligible Illinois children starting at birth.

This guide explains Part B of the Individuals with Disabilities Education Act (IDEA) special education laws, procedures, rights and responsibilities for parents, school professionals, and others.

A workbook designed as a resource and a planner for parents as their child transitions from Early Intervention services.

Illinois State Board of Education/Early Childhood Education 

This resource goes directly to the Early Childhood (EC) Education page of the Illinois State Board of Education (ISBE), where resources include information on EC Special Education Programs, Preschool for All, Prevention initiatives, and various other resources pertinent to the 3-5 year old population.

Transition Advocacy

  • Family Matters Parent Training and Information Center (PTIC), serves the entire state of Illinois outside of the Chicago metro area and provides information, referral, linkages, and training to parents, students with disabilities, and special education professionals. Also offered are free workshops on special education rights and responsibilities and related topics. www.fmptic.org
  • Family Resource Center on Disabilities Parent Training and Information Center (PTIC), serves the Chicago metro area and provides information, referral, linkages, and training to parents, students with disabilities, and special education professionals. Also offered are free workshops on special education rights and responsibilities and related topics. www.frcd.org
  • Family Support Network of Illinois' mission is to unify individuals with disabilities and their families to advocate for funding, services, and community resources.
